Do You Know of a Child?
If
you know of a child, or if the child is in
your family and meets the following criteria for
eligibility of having a dream fulfilled:
- The child must be between the ages of 3 and 18.
- The
child must have a critical illness as
determined by his or her physician and our
screening committee.
- The child
cannot have received a dream, wish or gift
from any other similar organization.
- The dream
must be the child's and not that of his or
her parents or another concerned
party. The child must be able to communicate the dream to a Dream Factory representative.
Please fill out a
Dream Application, and then you can either print
and mail the completed form by selecting file and
print, or just submit online.
